Workers’ Compensation

State law requires that all alleged workers’ claims involving five or more days of disability (indemnity claims) be reported by the employer with seven business days of the fifth disability day.  Indemnity claims in the category should be reported to the DIA and to MEGA.  Best practice also requires that all incidents alleged and all medical only claims be reported to MEGA for processing.  This link provides access (access rights are required for this secure site) for members to report claims via our web-based system directly to MEGA and to create the state required Employers’ First Report of Injury or Fatality (Form 101) for filing with the DIA.  Our system will assign a claim number as soon as the claim is submitted to MEGA.

The above link is a direct portal to MEGA’s claim reporting system and claim adjudication system.  This is a secured site and requires that member users have pre-determined access rights.  The link should be used to report workers’ compensation claims to MEGA and to prepare the state required Employers’ First Report of Injury or Fatality (Form 101).  Access rights can be granted for reporting of claims only or for management level employees to access all claim related information including adjuster notes, transactional history, financial analysis, and loss analysis.  Information is exportable to other software for use in creating internal reports. 


For those who prefer not to utilize a web-based reporting system, the attached documentation provides filing instructions and forms to be used to report workers’ compensation claims to MEGA and the DIA by facsimile or by regular mail.

We are also providing a link to the DIA’s information on How and When to File a Report of Injury

 We are also attaching a Medical Authorization form to be used by MEGA to obtain medical information on behalf of a claimant in the processing of a claim.  The form can be printed and a signature obtained from the injured employee for forwarding to MEGA in order to expedite claim handling.  If the authorization is not executed at the time the claim is reported, MEGA will pursue it directly from the injured party in the course of adjudicating the claim.   

 

Property & Liability

As with all claims, prompt and proper reporting of losses is essential to controlling claim costs. MEGA has established a process for reporting property & liability losses. The documents attached here can be used to report losses to MEGA.
