MEGA requires the following information in order to process quotation for various lines of insurance requested.  The information is available in electronic format.  Feel free to contact us for further information. 

1. Completed MEGA Membership Application
    

2. Depending on lines of insurance requested, MEGA requires updated schedules on property values, vehicles, student count, payroll, Fire & Police personnel & payroll, and employee concentration detail (see application for further detail).
    

3. Historical Loss Data - 4 complete years plus current year (value within last 6 months)
    

4. Proof of current experience modification factor (Workers' Compensation)
    

5. Loss detail on all worker's compensation losses in excess of $50K incurred
    

6. Supplemental Application Information on any out of ordinary risk
               (To be requested as needed)
